The Request form Editor offers six question types. Use this reference to choose the right type and configure it.

Subject
The request's title. Every form has exactly one Subject question, and it's always required.
Placeholder: up to 120 characters, for example "Briefly summarize your request"
Maximum length: how many characters customers can enter
Text
Collects a written answer.
Text type: Short text for a single line, or Long text for a paragraph
Placeholder: up to 120 characters
Minimum length and Maximum length: optional whole numbers. The minimum can't be greater than the maximum.
Use it for: descriptions, steps to reproduce, account names.
Number
Collects a numeric answer.

Minimum and Maximum: optional limits
Use it for: order numbers, number of affected users, quantities.
Selector
Lets customers choose from a list of options.
Selection mode: Single select or Multi-select
Choices: at least 2 and up to 50 options. Each option is up to 100 characters and must be unique.
Minimum selections and Maximum selections: for multi-select only. They can't be more than the number of options.
Use it for: impact, product area, browser, plan.
Date Picker
Collects a date. Answers use the format YYYY-MM-DD.
Use it for: when an issue started, a deadline, an event date.
File Upload
Lets customers attach files.
Allowed file types: choose at least one of PNG, JPG, JPEG, WEBP, MP4, MOV, PDF, and TXT
Maximum file size: 1 to 25 MB per file
Maximum file count: 1 to 10 files
Use it for: screenshots, screen recordings, invoices, log files.
The default attachment question allows PNG, JPG, JPEG, WEBP, PDF, and TXT files, up to 5 files of 10 MB each.
Changing a question's type
A question's answers are stored against it over time. If you need a different type, add a new question rather than repurposing an old one, and delete the old question. Existing requests keep their original answers.
